I will provide,
1. Basic bug report & screenshots
3. Steps performed for bugs(in screenshots and only for premium package)

The further details are mentioned below.

1. On Basic & Standard Package, all the details related to package will be reported along with the screenshots and the basic report will be maintained for each bug. Bugs will be reported using Excel sheet. The Basic report will include the short description of the test case and the result will be provided as screenshot.

3. For Premium package clients, I've some more to offer, UI testing along with Sanity will be performed.
The screenshots of premium will include steps performed to do this will be clearly mentioned in the screenshots. Functional specification document MUST be provided, as it would be great to use it to achieve maximum results (after testing).
